Patti has been a social worker for an amazing 49 years, and for the past 20 years, she has been an integral part of the team at Home Care Resources. Her experience, knowledge, and compassion have made a lasting impact on countless clients and families.
Patti brings a unique level of expertise by going into homes, facilities, and communities to assess the level of care needed for our clients and develop individualized care plans. Unlike many home care agencies, Patti completes comprehensive bio-physical and social assessments, ensuring we meet the needs of each client we serve.
She consistently goes above and beyond by connecting clients and families with valuable resources that enhance both quality of life and safety. Her recommendations often include medical equipment providers, attorneys, fiduciaries, care managers, memory care communities, palliative and hospice care, home health services, and placement specialists.
Patti has dedicated much of her career to geriatric services since 2006, and prior to that, worked in medical detox and dual-diagnosis care.
Her compassion, expertise, and commitment to helping others embody the very heart of social work.
